Nursing Assistant Application Guide
The Nursing Assistant Course is an approved Oregon State Board of Nursing certificate training program. Upon successful completion of this course, students will be able to take the state certification exam for the Oregon State Board of Nursing Certified Nursing Assistant Licensure.
The Nursing Assistant Skills Labs are scheduled to take place at 91重口's Healthcare Occupations Center in Lebanon. Students completing the first phase of the course will be assigned to clinical rotations at an approved Oregon State Board of Nursing skilled nursing facility located in Lebanon as well.

Class Selection Process
Applicants submitting an online application and completing all requirements before the decision deadline will be qualified for consideration to the course. Seats will be assigned by the date an applicant completes the posted requirements.
If a qualified applicant is not seated in a class, the applicant will be designated as an alternate. Should seats become available, alternates will be offered seats based on the day their alternate status was assigned. If the alternate is not offered a seat, the applicant will need to reapply to a future term.

Application Requirements
Applicants must upload the following documents to qualify an application.
Proof of Hepatitis B Vaccination:
- Any documentation indicating the applicant has completed or is in the process of obtaining immunity to Hepatitis B. Upon admission to the program, applicants will be required to submit official proof through our compliance tracker.
Applicants whose application has been approved will then be required to submit the
following immunization documentation.
Tuberculosis:
- Tuberculosis test (TB) negative results report, Quantiferon Gold blood test, two-step skin test, or a negative chest x-ray
Varicella:
- Two vaccines or an antibody surface titer with a positive result
Tdap:
- Vaccination must have been administered within ten years prior to the first day of the course
MMR:
- Two vaccines or an antibody surface titer with a positive result
Hep B:
- Completion of two-vaccine series (e.g. Heplisav-B), a three-vaccine series, or an antibody surface titer with a positive result
Note: only after the application is accepted and approved will applicants be provided with the information necessary to set up and pay for a required immunization tracking compliance account, drug screen, and background check. Costs for this service are non-refundable and purchase does not imply final acceptance into the course.
Admission Limitations
Applicants should be aware of the following admission limitations. Failure to comply may result in application denial and/or admission eligibility forfeiture.
Application Completion
Applicants must submit proof of completing all minimum application requirements by the deadlines given to be considered for admission to this program.
Previously Admitted Students
Applicants who have previously been admitted to this program are no longer eligible to apply if they exited the program based on safety concerns.
Clinical Placement Eligibility
Applicants must be in good standing with this program鈥檚 clinical site partners. Applicants who are ineligible due to a violation of the Oregon Nurse Practice Act or who have been denied access privileges to one or more of the program鈥檚 clinical partner sites will not be eligible to participate in this program. Meet with the Nursing Program Administrator prior to applying to this program if you have concerns regarding your placement potential.
Holds
Applicants must have no registration holds at 91重口 during the entire application and admissions process to be considered for admission to this program. Financial holds must be addressed through the 91重口 Business Office prior to program admission. Note: new student holds (no previous 91重口 credits or registration) are the only exception.
Statements of Understanding and Compliance
- 91重口's Students Rights and Responsibilities Handbook, including the Standard of Student Conduct.
- Applicants must comply with all application criteria and deadlines.
- Communication from 91重口 regarding your program application will be sent to the email submitted on the application.
- Accepted students will have their social security number (SSN) matched against the US Department of Health & Human Services list of individuals who are barred from any participation in federally funded health programs. If on the list, you will not be eligible to participate in this program.
- Admitted students will be required to cooperate with the drug test policies of any clinical as a condition for continued enrollment in this program and/or related academic courses. A student may be required to comply with the non-91重口 site's policy. Testing may be random and unannounced or conducted when there is reasonable suspicion that performance and/or work behavior may be the result of the presence of a drug. The presence, as determined by the program's test procedures, of prescription or non-prescription drugs, controlled substances, or cannabis may result in immediate dismissal from the work site and disenrollment from the course. The student may also be subject to appropriate disciplinary action for violating 91重口鈥檚 Student Standards of Conduct.
